In order to play a spring sport, students must have a physical dated on or after June 1, 2019.
Spring physical paperwork and recertification paperwork for fall/winter athletes/team managers will be available beginning on 2/3/20 at SMS/SHS.
If you participated in a fall/winter sport or were a fall/winter team manager, you do not need a new physical, however, you do need to complete a Recertification form to participate in a spring sport. Note, if injured in a previous season, you must be released by your treating doctor

A PHYSICAL CANNOT BE GIVEN WITHOUT THE CONSENT OF A PARENT OR GUARDIAN, REGARDLESS OF STUDENT AGE. PLEASE SIGN THE PARENTAL CONSENT SECTION SO THAT YOUR CHILD MAY RECEIVE A PHYSICAL ON FEB. 13.
