Greetings Salisbury Community! CVS pharmacy and the STSD nurses are holding a community flu shot clinic at Salisbury High School on 10/29/20 from 10AM-1PM. Vaccines are available for persons aged 3 and up. Please see the attached flyer for information! Everyone who participates and gets their shot at these events will be awarded a $5 Target gift card! If interested in receiving a flu shot, please read the following instructions: You must make an appointment to participate. To make your appointment, sign up at the link below. https://www.signupgenius.com/go/10c0d4ea9af2faafec43-salisbury If unsure how to make an appointment using sign up genius, call the school nurse to make an appointment (contact information at the bottom of this email). Please make one appointment for each person participating. Print out the attached flu consent form . Complete the form and bring it to the clinic. Make sure to include your insurance information. If you don't have a printer, forms will be available when you arrive. CVS accepts most commercial insurance plans, medicare and medicaid (gateway, amerihealth, aetna better health). CVS normally charges around $25 per flu shot for those without insurance. Upon arrival for your appointment, pull up to the front entrance of SHS, park your car, and go to the main entrance.The vaccines will be given under the roof. Appointments are limited, so sign up today!
