SMS/SHS Fall Athletic Physical Options
If you are currently in grades 6-11 and are looking to play or team manage an SMS/SHS PIAA sport, you will need to get an athletic physical on or after May 1, 2024.
This is not for SYA. Please also note that the following physical dates are NOT options for marching band members/twirlers unless these students are currently in grades 6-11 AND plan to also play a fall, winter or spring sport. We will provide another opportunity
for twirlers/marching band members to receive physicals.
Salisbury will offer FREE athletic physicals on Tuesday, June 4 at Salisbury High School between 5 and 7 p.m. See the signup genius below.

Please also note that St. Luke’s will be offering FREE athletic physicals on June 1, June 25, July 16 and July 30 at Coca Cola Park in Allentown. See the link below.

You may also receive a physical from another approved health care provider if you are unable to attend physicals at SHS or Coca Cola Park.
Please bring PIAA Section 5 (completed) and Section 6 (complete top 3 lines) forms to your physicals.
